Outreach services for the elderly – home visits – are offered by the open and outpatient elderly care services of the city of Bremen.
With visiting and companionship services, we aim to support elderly people who live at home and have few social contacts. The Senator for Labor, Social Affairs, Youth and Integration supports our work in the Bremen districts of Mitte, Gröpelingen, Hemelingen, Neustadt, Obervieland and Vahr.
We are a diverse group of social organizations, institutions, and associations. Since 2009, we have been visiting, supporting, and advising older people. Since 2023, we have offered expanded services at two locations, in Neustadt and Gröpelingen, through the new pilot project "Outreach Services for the Elderly Plus" (AAA+).
Our offers
The "Outreach Services for the Elderly - Home Visits" program is organized by social service providers and carried out by volunteers. A full-time coordinator serves as the contact person for the service in the neighborhood.
- We will provide information about our work in a personal conversation
- We will arrange a regular contact schedule with you
- Upon request, we will visit you at home once a week
- We can accompany you to events, doctor's appointments, shopping trips, or even walks
- If you wish, we can advise you on further assistance and support services
- We are here for you – free of charge!
The scope of joint activities can be tailored to your personal wishes and agreed upon with us – however, housekeeping and caregiving activities are generally excluded.

Do you want to get involved in a meaningful way and connect with people from your neighborhood?
- They meet an elderly person once a week
- They keep him company at home or on walks
- If you wish, you can attend joint events or accompany them to doctor's appointments
- In this way you can support community life in the neighborhood and actively work against exclusion and isolation.
- Join a special visiting service that will give you touching, unusual and unique insights!
What we offer you

- You can manage your time freely
- You can participate in regular volunteer meetings and come into contact with dedicated people
- They will be invited to further training courses
- Tickets for buses and trams will be refunded
- You can spend some of your free time with older people. You'll engage in conversation and give and experience joy and appreciation for their commendable commitment.
