End-of-life care in one's own home
Many terminally ill people wish to live in their familiar surroundings for as long as possible. We at the Bremen Outpatient Palliative Care Service provide care for people with an incurable, advanced illness in their own homes. Our support is provided in close consultation and cooperation with the family doctor and outpatient nursing services.
Our aim is to achieve the highest possible quality of life and self-determination for our patients. We help alleviate complaints and symptoms, treat them, and provide ongoing support to those affected and their families.
How does the support provided by the outpatient palliative care service work?
The support provided by our palliative care team is tailored to each individual. Therefore, in an initial, detailed consultation at your home, we will work with you and your family to determine the necessary support measures. The treatment plan is based on your wishes and needs and is continuously adjusted. Depending on your requirements, we will arrange daily or several times a week home visits.
We offer a 24-hour emergency hotline for our patients. You will receive this emergency number at the start of your treatment.
During our palliative medical and nursing care, we work closely with general practitioners or specialists, outpatient nursing services, hospice services, social services, and other care providers, depending on the patient's individual situation.
Should patients under our care wish to be admitted to a hospice or other inpatient facility, we will assist them with the organization.
Who receives specialized outpatient palliative care?
Patients with advanced illness and a limited life expectancy can receive outpatient palliative care. This entitlement is not tied to a specific underlying disease or diagnosis and can also be provided in nursing homes.
The costs of specialized outpatient palliative care (SAPV) are covered by statutory health insurance. It can be prescribed by a general practitioner, specialist, or attending hospital physician.
Our goal is to enable people who are ill to spend their final days in their familiar surroundings, as much as possible according to their wishes and with relief from debilitating symptoms. Family members are an important pillar in this process, and we gladly involve them, but even single individuals can find their way to us during this phase of their lives.

- Pain therapy, drug and non-drug treatment of distressing symptoms such as shortness of breath, nausea, constipation, vomiting, anxiety or insomnia.
