The Bremen Palliative Care Team provides care for seriously ill people in their homes, in all districts of Bremen.
The Bremen Palliative Care Team ensures that patients can spend their final days as self-determinedly and according to their wishes as possible. Palliative care nurses, physicians, and chaplains provide support where care from general practitioners and home care services is no longer sufficient. Those affected and their families will always find a listening ear. Specialized outpatient palliative care (SAPV) is covered by statutory and private health insurance. A prescription from a general practitioner, specialist, or hospital physician is required.
Offers:
- Pain therapy and treatment of distressing symptoms such as shortness of breath, nausea or anxiety
- Wound management
- Advice on nutrition and fluid intake
- Support for relatives with questions about care and treatment
- Organization of resources
- Psychosocial support for patients and their families
- Pastoral care

The care is provided upon request. Patients and their families can initiate it by asking their family doctor or the treating hospital for a prescription for specialized outpatient palliative care (SAPV). For questions, the Bremen Palliative Care Team can be reached at +49 421 247 11 433.
