We offer individualized support services to enable you to live independently at home for as long as possible. We would be happy to connect you with other healthcare providers in Bremen or organize further assistance.
If needed, we can arrange for you to be accompanied to doctor's appointments or to an assessment by the Medical Service of the Health Insurance Funds (MDK) at your home. This way, we improve your quality of life!

Together with you, we develop targeted questions for important doctor's appointments.
We provide information on advance healthcare directives, living wills, and guardianship orders – both digital and traditional. Digital advance care planning has also become an indispensable component of security. We would be happy to explain its significance and relevance to your personal circumstances. We
are also committed to ensuring a dignified and individualized end of life (end-of-life care, palliative care). We will discuss this with you, approaching this often-taboo subject sensitively and giving appropriate consideration to mortality.
